Patient resources

We want you to have a great experience when you visit Campus Health Center (CHC) so we compiled helpful patient resources including forms, policies and health and safety resources both on and off-campus. To ensure a smooth visit please see the reminders below.

A few reminders

  • Schedule your appointment online. Complete the pre-check-in process and upload all needed documents prior to your appointment. Note: You will need to upload your insurance card. Please use your legal name.  
  • Activate your patient portal once you have your WSU Access ID. All test results will be uploaded to your patient portal.  
  • Out-of-network plans or HMOs may require a referral and/or prior authorization from your primary care provider for some services. Contact your insurance company for more information. For questions about insurance, visit our frequently asked questions.
  • If you are under 18, your parents should complete the Parental/Guardian Patient Enrollment and Consent Form. Bring the form to your first appointment.  

Need to cancel?

We ask that you cancel appointments as soon as possible so we can open that time slot for another student. You can easily cancel your appointment through our online appointment scheduling system or by calling 313-577-5041. If you do not show up for a scheduled appointment, you will be billed a no-show fee of $15.

You will receive a link to a patient satisfaction survey a few hours after your appointment, we encourage you to let us know how we did. If you have a compliment or a complaint that you would like to discuss, contact our Associate Chief Nursing Officer at 313-577-5120.
