Flu Clinic Reminders

1. After you schedule your appointment, check your wayne.edu email for your electronic flu vaccine consent form, they are emailed at 8 a.m.
2. Complete and submit your electronic flu vaccine consent form prior to your appointment
3. Double check the clinic location by referring to the Flu Clinic calendar above (Flu Clinic appointments are NOT at Campus Health Center)
4. Please bring your One Card, state ID, cleared Campus Daily Screener, and insurance card to your appointment
