CHC will be providing UDS (Urine Drug Screening) and TB Testing (T-Spot) to WSU College of Nursing students at Clinical Permit Day.

Clinical Permit Day:

Tuesday, May 5 and Friday, May 8 from 7:30am-3pm at the Campus Health Center, 5285 Anthony Wayne Drive.

Important Notes:

Prior to coming to Clinical Permit Day, you must:

  1. Create an account in the Campus Health Center Patient Portal.
  2. Complete all sections of the registration form, including insurance information.
    1. For detailed instructions on how to do this, please visit the Patient Portal Instruction Guide.
  3. Call your insurance company to see if they cover a T-Spot. If you have additional questions after calling your insurance, please call the front office at CHC at 313-577-5041.

CHC will bill your insurance for the cost of the TB test. If your insurance does not cover it, you will receive a bill in the mail. If you do not have insurance, you will be required to pay $64 for your TB test. This is the reduced rate for this test.

CHC will be conducting all Urine Drug Screening (UDS).  UDS are NOT billable to insurance. The cost of UDS is $25.00. Payment is required PRIOR to the test being conducted.

If you are paying for both tests, the total cost is $89.

Advance payment is STRONGLY encouraged and preferred.  Payment can be made online for both the UDS and TB test by clicking here.  Payment will be accepted day of with credit card only.

CHC will send your results by Friday, May 15th, 2020 via the Patient Portal.  It is the responsibility of the student to sign up for the Patient Portal to get your electronic results and submit the documentation to the College of Nursing.  CHC will not be providing paper copies or submitting any documentation for you.   • Government ID (driver’s license or passport)
  • Insurance Card (photocopy if you have one)
